Collecting Consent Information

FairConsent makes it simple to collect informed consent.
Follow these steps:

Step 1: Select Consent Type

Choose the type of consent you would like to record:

• Person: For adults or children and their legal guardians
  
• School or Institution: Select this option if you're recording consent for someone authorized to sign on behalf of others (e.g. a school director with legal guardian consents available)
  
• Learn how to record consent in schools / institutions

Step 2: Add personal information

Fill in the details for one of the following:

• - An adult individual
• - The legal guardian of the child(ren)
  - A person authorized to sign on behalf of others
  
  
  

Fields to Complete:

• Name: Enter the full name. Check "Use Pseudonym" if anonymity is preferred.
  Learn how to use pseudonyms
• Age: Provide the individual's age.
  
• Gender: Select Female, Male, or Diverse.
• Phone & Email (optional): Recommended for record-keeping and communication.
  
• Notes: Add relevant details, such as usage rights or additional context.
