Recording Consent for Children or Wards

When recording consent for children, ensure the consent of their guardian is recorded first.
Learn how to collect consent information

1. After Guardian Consent
Once the guardian has confirmed their consent, click on "Add Child or Ward".

2. Enter Each Person Under Guardianship Information
For each person, fill in:

• Name
• Age
• Gender
• Any relevant Notes (e.g., project-specific details).
  

Click on "Continue" to capture a photo of the child's or ward's and save their information.

3. Adding Multiple or Additional persons under guardianship

• To add more persons to the same guardian, click "Add Child or Ward" again and repeat the process for each person.
  
• If you forgot to add a child or ward or need to make changes, you can do so by editing the recorded data.
